Yr 7 and 8 Immunisation Forms

Please complete your Year 7 or 8 child’s immunisation form and send it signed back to the office.  You have an option to immunise or an option to not immunise.   The Yr 7 immunisation is a Boostrix – a boost against tetanus, diphtheria and whooping cough.  The Year 8 vaccination is the HPV (Human Papillomavirus Vaccine).
