School Bus Transport Information Form and School Transport Assistance

The Ministry of School Transport requires the following information so it can be entered
onto a National Database for all students who require School Bus Transport assistance. New
students enrolling and any students who change address within the district are now
required to apply for assistance and complete an application form regardless of whether
they will receive assistance in the form of conveyance allowance or a bus ride.

• Specialised School Transport Assistance (SESTA)
